You can choose from either SofLens Daily Disposables or PureVision 2 HD.  All you have to do is fill in the form, and bring it to your eye care professional.  If you are due for an eye exam, you will have to have that done first obviously, but if your prescription is current, you should be able to just redeem it!

This a great if your child needs some for back to school in a few months (I know, I know, it just ended!)  I'm not sure how many certificates are available to print yours now!
